Making an Appointment & Return Appointments
When making an appointment, please be ready with the following information:
- Name
- Date of Birth
- Reason for Appointment
- Insurance information
We have an appointment system which allows access to your Primary Care Provider. If an appointment is not available for the time you need, feel free to speak with one of our LPN's to assess your need and assist you. Sick calls and clinical advice will be reviewed by the nurse with a return call.
In addition to this, we will be starting a new appointment schedule to increase your access to appointments when you need them. Starting in 2012 providers may ask that your next appointment be placed in "Recall" which means you will be able to call in the future for that appointment.
Medication Refills
If you need a medication refilled, we ask that you call your pharmacy directly. Many times refills are available at the Pharmacy and if needed they will contact our office. We recommend calling 5 days in advance to avoid any delays.
If you take medications that are given to you as a sample or require a written prescription, please contact our office 5 days in advance and be ready with the following information;
- Name
- Date of Birth
- Medication Name
- Phone Number
After Office Hours Access
If it is after hours and you have an emergent question (for Behavioral, Medical or Dental) we have a Provider on Call. Simply call the office number and the answering service will page the On Call Provider whom will call you back. When calling have the following information ready:
- Name
- Date of Birth
- Your Provider's Name
- Phone Number
- What the Problem is
