Our Health Education Department assists patients and community members with getting insurance coverage. Care Coordinators at Generations are available to meet with both patients and community members who do not have health coverage or have limited health coverage benefits.
Health Insurance Eligibility Screening and Application Services for the Uninsured and Underinsured:
Generations Care Coordinators will assist you with applying for the state funded HUSKY Health program. (HUSKY Health is the State of Connecticut’s public health coverage program for eligible children, parents, relative caregivers, senior citizens, individuals with disabilities, adults without children, and pregnant women.) Children up to age 19 may qualify regardless of family income level. The Care Coordinators will work with each individual and family to determine which program is a good fit and help to complete the application. They can also grant immediate HUSKY insurance coverage for eligible children, pregnant women and for the HUSKY Limited Family Planning benefits, on behalf of the Department of Social Services.
In addition, Care Coordinators are knowledgeable about other health programs and funds for those without health coverage who may not be eligible for HUSKY Health and will link them to resources for medication assistance, preventive health screening programs or other health services they may need.
PRIVATE INSURANCE-ACCESS HEALTH CT
If you have had a change of life circumstance such as birth of a child, marriage, divorce, loss of or change in employment, etc. and you do not qualify for HUSKY or Medicaid then Generations can help you find a private insurance plan through Access Health CT.
WHAT IS ACCESS HEALTH CT?
Access Health CT is Connecticut’s official health insurance marketplace. It offers small employers a range of qualified health care coverage options for health insurance. You can get the coverage you need online, over the phone, or in person.

INFORMATION YOU WILL NEED TO SIGN UP FOR HEALTH INSURANCE:
- Birth dates for all family members who need coverage.
- Social Security Numbers for family members who need coverage (or document numbers for legal immigrants)
- Citizenship or immigration status
- Tax returns for previous years
- Employer and income information for every member of your household who needs coverage (for example, from pay stubs of W-2 forms – wages and tax statements)
- Health care coverage information (policy numbers for any current health insurance plans covering member of your household)

What is SNAP?
SNAP is a Supplemental Nutrition Assistance Program. SNAP helps individuals and families buy food. It used to be called the Food Stamp program. Anyone can apply for SNAP.
How Much Can I Get?
The amount you receive depends on your income, your expenses and your family size. You have to prove that you are a legal resident to be eligible for SNAP. Applying for and getting SNAP will not hurt your chances to become a citizen.
How Do I Apply?
Generations employs SNAP workers that can tell you what types of paperwork will be needed like proof of identify and pay stubs and will help you apply.
Once you finish your application, the state Department of Social Services will call you for an interview. The interview MUS BE COMPLETED in order for you to receive SNAP.
When and How Can I Use SNAP?
Once approved, you will receive your SNAP EBT card. Most grocery stores, corner stores and even some farmers’ markets accept SNAP. Look for SNAP Logo.
