Insurance
We accept Medicare and HUSKY Health (Medicaid) as well as many commercial health insurance plans. With insurance, coverage and any out-of-pocket costs for your visit are based upon many factors, such as services provided, your insurance status, your insurance policy’s benefits, deductibles, co-pays or co-insurance fees.
We recommend that you review your health insurance plan booklet or call your insurance company representative before you visit, to make sure you understand your specific policy benefits and provider network requirements. It’s important to know who you have chosen as your Primary Care Physician, and to update that if necessary before your appointment.
Failure to follow your plan requirements may result in greater out-of-pocket expenses for you, so it’s really important to stay up to date with your insurance company’s policies.

Network coverage does not guarantee that all services rendered by Generations Family Health Center will be covered. Each plan has specific benefit allowances that vary based on plan type, service and provider. Please consult your plan benefits to determine coverage of services.
Out Of Network
If your insurance is not listed above, we may not be in network with your insurance company. We will accept and bill your insurance on your behalf as long as your insurance accepts claims from us. You will be responsible for any non-covered services or denied claims if your plan is out of network. Please call your insurance company if you are unsure of your benefits.
Sliding Fee Scale
Generations Family Health Center provides services regardless of a person’s ability to pay, with insurance or without insurance. If you do not have insurance, you’re underinsured (your insurance doesn’t cover everything you need it to), or if you have high out-of-pocket costs, Generations offers a Sliding Fee Discount program. Those meeting specific income qualifications may be eligible for a discount on medical, dental or behavioral health services rendered at Generations Family Health Center regardless of insurance coverage. If you qualify, visit fees will be based on a sliding scale using income and household size.
For our insured patients this could mean a lower out-of-pocket expense on your co-pays, coinsurance and/or deductibles.

Payment Plans
If you’re unable to pay your account balance, we offer payment plans. This can be set up via your online check in or by contacting our Billing Department at (860) 450-7471.
Prescription Assistance
If you have concerns about paying for your prescriptions, we have a Prescription Assistance Program that may be able to help. Please talk to your provider about your concerns, and they can help you access discounted prescriptions.
Good Faith Estimate
All patients have a right to receive a “Good Faith Estimate” providing an estimate of your expected medical care costs.
A Good Faith Estimate will be generated automatically for all our self-pay or uninsured patients for appointments scheduled more than three days in advance.
Patients who schedule fewer than three days in advance or those who would like a Good Faith Estimate for any reason can request one from the office for any of our medical, dental or behavioral health services.
The Good Faith Estimate provided by Generations Family Health Center will be rendered to the best of our ability based on how the appointment is scheduled and the information we have at the time of generating the form. If new or acute concerns arise that require additional labs or tests, further charges may also occur, in which case the patient can inquire as to the additional costs.
If the bill for your service is greater than the Good Faith Estimate by $400 or more, you can dispute the bill by contacting Generations Billing Department. All disputes will be reviewed, accessed and addressed in a timely manner with patients.

Remember …
- If you have insurance, please make sure you bring your insurance card with you to every appointment.
- You must pay your copay at each visit.
- Patients are responsible for paying any unpaid balance for services. This includes the sliding scale fee, co-pays, co-insurance, deductibles, and other services not covered by insurance.
