Making an appointment and return appointments:
When making an appointment, please be ready with the following information:
- Name
- Date of Birth
- Reason for Appointment
- Insurance Information (if you are insured)
Medical appointments:
We have an appointment system that allows access to your Primary Care Provider. If an appointment is not available for the time you need, feel free to speak with one of our nurses to assess your need and assist you.
Medication refills:
If you need a medication refilled, we ask that you call your pharmacy directly. Many times, refills are available at the pharmacy and if needed they will contact our office. We recommend calling five days in advance to avoid any delays.
If you take medications that are given to you as a sample or require a written prescription, please contact our office five days in advance and be ready with the following information:
- Name
- Date of Birth
- Medication Name
- Pharmacy Name
- Pharmacy Phone Number
After hours access:
If it is after hours and you have an emergent question (for Behavioral, Dental or Medical), we have a health care provider on call. Simply call the office number and the answering service will page the On-Call Provider who will call you back. When calling have the following ready:
- Name
- Date of Birth
- Your Provider’s Name
- Your Phone Number
- What the Problem Is
