Putnam School-Based Health Center 

Generations Family Health Center’s Putnam School-Based Health Center is here to help you and your child. The Putnam School-Based Health Center (SBHC) is a partnership between Generations Family Health Center and Putnam Public Schools providing Behavioral Health services to students at their schools or via video if they are unable to do in-person visits.

Who is eligible?

Putnam Public School Students Pre-K – 12th grade.

These services can be in addition to the services students receive from existing providers. 

What services are available this year?

Behavioral Health Services: in-person and via video during the 2024-2025 school year. Therapeutic group sessions are offered at the School-Based Health Center throughout the school year.

Note: Generations Mobile Health Unit will soon provide dental services at schools in eastern Connecticut, including Putnam Public Schools

Payment:

No student will be refused services based on inability to pay. These services can be in addition to the services you receive from existing providers.

Insured/Uninsured:  

  • The School-Based Health Center accepts most insurances. For those students with insurance, their insurance will be billed.
  • Eligible families may apply for a sliding fee scale discount based on family size and income.
  • For the uninsured, we work with families to screen for insurance eligibility. We can work with you on a payment plan.
