Diabetes Support Program (DSP)
Virtual Coaching to Help You Take Control of Your Diabetes
The Diabetes Support Program (DSP) is a fully online FREE coaching program for eligible Generations patients living with diabetes. DSP helps you build practical skills, confidence, and daily routines to better manage your health, from the comfort of your home.
Why Join DSP?
- Completely Online – Participate using your computer or smartphone through the secure HALT 360 app
- Personalized Health Coaching – Work one-on-one with a dedicated health coach who helps you set goals, track progress, and stay motivated
- Interactive Learning – Weekly rotating lessons, activities, and educational modules
- Community Support – Connect with others on a similar journey for encouragement and accountability
- Photo Meal Journaling – Share photos of meals and receive personalized feedback from your coach
- Health Tracking Tools – Log blood sugar, physical activity, and more while seeing progress in real time
Who Is Eligible?
- Current Generations Family Health Center patients
- Diagnosed with diabetes or pre-diabetes
- Access to a smartphone or computer
Healthy Weight Clinic (HWC)

Small Steps. Big Changes. Healthier Families.
The Healthy Weight Clinic is a year-long, family-centered program for children and teens ages 2–18 whose weight is above the healthy range for their age and sex. This supportive program helps families create realistic, lasting habits without diets or shame.
This Is Not a Diet Program
The Healthy Weight Clinic focuses on:
- Building healthy eating and activity habits
- Making small, manageable lifestyle changes
- Addressing medical concerns related to weight
- Increasing confidence and healthy routines
- Connecting families with helpful community resources
Who Is Eligible?
Children and teens who:
- Are ages 2–18
- Receive care at Generations Family Health Center
- Have a caregiver willing to participate in visits and goal setting
Family involvement is key to success.
What to Expect
Every family receives an individualized plan tailored to their child, routines, and lifestyle. Support may include:
- Individual Family Visits – Medical care and personalized goal setting at our Putnam location.
- Telephone Check-Ins – Ongoing support between visits
- Group Sessions – Learn alongside other families at our Putnam location.
- Flexible Options – In-person or virtual visits
Visits start every few weeks and become less frequent as healthy habits grow. Support continues all year.
Your First Visit
During your first appointment, you will:
- Meet the Healthy Weight Clinic team
- Review your child’s health history
- Set family-centered goals
- Create a personalized plan for your child and household
