Generations Family Health Center’s Putnam School-Based Health Center is here to help you and your child. The Putnam School Based Health Center (SBHC) is a partnership between Generations Family Health Center and Putnam Public Schools providing Behavioral Health services* to students right at their schools or via phone or video if they are unable to do in-person visits.

7:30 am to 4:00 pm
Note: Due to space issues at the High School, we will only be offering in-person services at the Middle School for all students including High Schoolers this school year.

These services can be in addition to the services you receive from existing providers.
How do we get services?
Please click on this link to complete the REQUIRED registration paperwork. Your child must be registered at the beginning of each new school year to be seen. The registration paperwork can easily be filled out and submitted on a computer or smartphone. Forms with an asterisk* are required.
Registration form:
Behavioral Health (BH) Informed Consent:
This form must be completed to consent to treatment for your child.
HIPAA Acknowledgement:
This form must be completed to confirm that you received a Notice of GFHC Privacy Practices (attached above for your review).
BH Agreement between Outpatient and Patient or Parent/Guardian:
This form must be completed and signed by the child and parent/guardian. It informs the child about the Behavioral Health Department and their rights as patients.
Ongoing Communication of Protected Health Information – SBHC:
Please complete this form to give us permission to communicate with someone other than the child’s parent/guardian. This might include a relative, the school nurse, or someone else involved in your child’s care. You may also use this form to restrict information that is released to anyone.
Consent by Proxy for Pediatric Care – SBHC:
Please complete this form if you wish to allow someone other than the parent or guardian to accompany your child to appointments.
Primary Care Provider Communication – SBHC:
Please complete this form if you wish to allow us to communicate with the child’s primary care physician.

Insured/Uninsured:
- The School-Based Health Center accepts most insurances. For those students with insurance, their insurance will be billed.
- Eligible families may apply for a sliding fee scale discount based on family size and income.
- For the uninsured, we will work with families to screen for insurance eligibility. We will work with you on a payment plan.
*Due to the pandemic, dental services are not available at the School-Based Health Center. We are scheduling children at our dental clinic at our Putnam site at 202 Pomfret Street.
