How to sign up for your MyHealthRecord
- Provide us with your e-mail so that we can send you a secure link to create your Generations MyHealthRecord account.
- Follow the Register Here link in the e-mail with subject line Register with Generations Family Health Center before the 14-day expiration date.
- To complete your registration, you will be asked to enter your first and last name, date of birth and zip code exactly as Generations has it on file for you and follow the instructions on screen to complete your registration.

General Use
How do I request access to Generations MyHealthRecord.com?
If your email is already on file, message us through the above button, and we’ll send you a registration link. If we don’t have your email in our system, you’ll need to visit our front desk in person to provide your email and complete the registration process.
Can I update my contact information in my Generations MyHealthRecord account?
Yes! You can update your phone number, email, address and Insurance directly in your Generations MyHealthRecord account.
How soon can I see my test results?
Most lab results are available in your Generations MyHealthRecord account as soon as they are released by the lab. However, some results may require a clinician’s review before being shared.
Can I use the portal to request a referral?
Yes, you can request referrals through your Generations MyHealthRecord account. However, some referrals may require additional information from your clinician before being processed.
Can I use my Generations MyHealthRecord account to share my health information with providers outside of Generations?
Yes, you can download, print out or email your records to any provider or email of your choice, anytime.
Can I use my Generations MyHealthRecord account to get my immunization records?
Yes, you can access and share your complete immunization record.

If my child is under 13 can I access their Generations medical record?
Yes, parents or legal guardians can request access to their child’s medical records.
Can I see my 13–17-year-old’s records?
State privacy regulations protect certain health information for minors aged 13–17. With your child’s consent and their completion of appropriate authorization forms, access will then be granted.
Can I access my spouse’s or elderly parent’s records through MyHealthRecord?
Yes, with the patient’s consent and their completion of appropriate authorization forms, access will then be granted.
Technical Issues
What if I forget my username or password?
If you forget your login credentials, click the “Forgot Username” or “Forgot Password” link on the login page and follow the instructions to reset your information.
Is Generations MyHealthRecord account secure?
Yes, Generations MyHealthRecord is encrypted and follows strict security guidelines to protect your personal health information.
